Willow review

Price transparency

What is disclosed before intake, not a clinical rating.

Willow is an online prescription service offering compounded semaglutide and tirzepatide as injections or dissolving tablets, with physician messaging and shipping bundled into its standard program. Semaglutide starts at $299 a month and tirzepatide at $399, but higher doses can cost more. Its separate supplement shop adds a gut-health connection; buying a probiotic is not the same as joining its medical program.

The verdict

Willow shows its price before the medical intake, but leaves gaps, such as what the advertised price includes or which pharmacy fills the prescription. It ranks 28 of 65 on price transparency.

The score beside Willow's name rates how much it tells you before the medical intake — the price, the pharmacy and the terms — not how cheap it is.

What do you get with Willow?

  • Online health-history review by a licensed physician, with a prescription only if treatment is appropriate
  • Compounded weekly semaglutide or tirzepatide injections, or daily sublingual tablets, depending on the prescription
  • Medication, physician access, care-team support and shipping in the standard treatment subscription
  • Ongoing portal messaging and follow-up; Willow says patients typically keep the same physician, subject to staff availability
  • A separate retail shop offering GLP-1 Microbiome+ and Stomach Soothe supplements; these are not prescription medicines or automatically included program benefits

How does Willow work?

  1. Check your location and eligibility

Complete the assessment and provide your health history. The program is for eligible adults; Willow currently excludes people with diabetes, although it considers people with prediabetes or hypertension.

  2. Review the prescription and total

A physician decides whether treatment is suitable. Confirm the formulation, prescribed dose, monthly charge, pharmacy and next billing date before proceeding.

  3. Allow time for dispensing

The FAQ estimates 2–4 business days for pharmacy processing and another 2–3 days for delivery after shipment. A two-day shipping headline is not a two-day promise from signup.

  4. Follow up before renewal

Use the portal to discuss response or side effects and confirm any dose-related price change. The subscription renews automatically; cancellation has a processing cutoff.

How much does Willow cost?

Semaglutide

From $299/mo: compounded injection through listed 3.5 mg or sublingual tablets; 7 mg injection $399. Standard care/shipping included; outside-pharmacy costs differ.

Tirzepatide

From $399/mo: compounded injection at listed 2.125–6.375 mg; higher tiers $499–$549. Sublingual tablets $399–$459. Standard care/shipping included.

Where it operates

Limited U.S. availability; FAQ and help-center lists disagree on Wyoming and Puerto Rico — confirm location and formulation

Willow's treatment price table lists semaglutide injections at $299 a month through 3.5 mg and $399 at 7 mg. Tirzepatide injections cost $399 at the listed 2.125, 4.25 and 6.375 mg tiers, $499 at 8.5 and 11 mg, $524 at 13.5 mg and $549 at 17 mg. These are the seller's dose labels, not a recommendation or an equivalence to branded doses.

  • Semaglutide dissolving tablets are listed at $299 monthly across doses. Tirzepatide tablets cost $399 at 6 or 10 mg and $459 at 20 mg. The combination-tablet prices are $399 or $459, depending on strength.
  • The standard program includes medication, evaluation, physician access, support and shipping. Choosing an outside pharmacy instead introduces a platform fee and separate medication payment; no comparable total is supplied for that route.
  • The help article's general $199 starting headline includes NAD+, a different treatment. It is not a semaglutide or tirzepatide price. Willow does not bill insurance directly.
  • To avoid the next charge, request cancellation through the care team at least two business days before processing. The FAQ offers a full refund before the prescription goes to the pharmacy; requests after the cutoff apply to the next cycle.
  • Supplement purchases have separate checkout and subscription terms. Microbiome+ is listed at $59 for a three-month purchase or $50.15 every three months on subscription, not as a monthly prescription fee.

Who is Willow best for?

A good fit if

  • Eligible cash-pay patients who want medication and messaging in one standard subscription
  • People comparing published dose tiers rather than assuming one price lasts throughout treatment
  • Readers who want to distinguish prescription care from a provider's optional gut-support products

Look elsewhere if

  • People seeking FDA-approved branded Wegovy or Zepbound through insurance
  • Anyone assuming a compounded dissolving tablet is interchangeable with an approved medicine
  • People with diabetes, whom Willow's published eligibility policy excludes
  • Shoppers requiring a fixed price at every dose or guaranteed nationwide availability

What are Willow's pros and cons?

Pros

  • The help center gives specific drug, formulation and dose prices rather than only a signup teaser
  • Physician messaging and shipping are included in the standard program
  • Injection and sublingual routes are described separately
  • The FAQ gives a concrete cancellation cutoff and distinguishes refunds before pharmacy submission

Cons

  • Higher prescribed doses can increase the recurring cost
  • The public FAQ and help-center location lists differ; confirm your state and exact treatment before paying
  • The main service pages refer to pharmacy partners without clearly naming the dispensing pharmacy for a prospective order
  • Compounded medicines are not FDA-approved; oral-dissolving formulations should not inherit claims from approved injectable products

The pharmacy behind the medicine

It does not name its pharmacy. Willow does not publicly disclose which compounding pharmacy fills its prescriptions. That is common and it is not an accusation — but it does mean the one thing you would most want to verify about a compounded medicine is the one thing you cannot.

A compounded drug is mixed to order by a pharmacy rather than manufactured and released as a finished product, so the FDA has not reviewed the specific preparation you are prescribed for safety, effectiveness or quality. That is true of every compounded program, including the highest-scoring ones.

What else do people ask about Willow?

Does Willow offer injections or tablets?

Its public pages describe weekly compounded injections and daily sublingual tablets for semaglutide and tirzepatide. The physician determines the prescription. Willow states that its oral-dissolving formulations are not FDA-approved; tablet and injection doses are not interchangeable.

Does every dose cost the same?

No. The semaglutide injection table rises from $299 to $399 at its highest listed tier. Tirzepatide injections range from $399 to $549 monthly, while tirzepatide tablets range from $399 to $459. Confirm the next month's prescribed strength and bill together.

Can I use insurance or my own pharmacy?

Willow does not directly bill insurance. Its FAQ allows an outside pharmacy able to fill the prescription as written, but a platform fee applies and you pay that pharmacy separately. Obtain a complete quote before treating it as equivalent to the bundled plan.

Is Willow available in every state?

No. Willow publishes a limited service list, and its FAQ and help-center lists do not fully agree. Confirm your current location and intended formulation with the company before enrollment, particularly if you are in Wyoming or Puerto Rico, which appear differently across those pages.

Are Microbiome+ and Stomach Soothe part of the prescription?

They are separate supplements sold through Willow's shop. Microbiome+ lists L. plantarum OM and XOS; Stomach Soothe lists ginger, peppermint, licorice/GutGard and vitamin B6. The sales pages do not establish that the finished products prevent prescription side effects or replace clinical follow-up.

How do I stop automatic renewal?

Message the care team at least two business days before your next processing date. The FAQ says cancellation before pharmacy submission qualifies for a refund, while a late request takes effect for the following cycle. Keep the confirmation and check which shipment the request covers.
