Price transparency
What is disclosed before intake, not a clinical rating.
Form Health makes the most sense if you want a clinician and registered dietitian involved in your weight care, especially when insurance can help pay for appointments. It offers FDA-approved medicines when appropriate, rather than compounded semaglutide or tirzepatide. Its $299 monthly self-pay plan buys the care program; medication and lab work cost extra.

The verdict
Form Health shows its price before the medical intake, but leaves gaps, such as what the advertised price includes or which pharmacy fills the prescription. It ranks 40 of 65 on price transparency.
The score beside Form Health's name rates how much it tells you before the medical intake — the price, the pharmacy and the terms — not how cheap it is.

How does Form Health work?
Confirm eligibility and benefits
Share your insurance details and health history. Form requires an adult BMI of at least 30, or 27 with weight-related risks, plus a primary care visit within the past 12 months.
Arrange your first visits
Complete onboarding, provide medical records and download the app before meeting your clinician and registered dietitian by video.
Agree on a treatment plan
Discuss food, activity, habits and any suitable medication. A prescription is a clinical decision, and insurance approval is a separate decision.
Continue care and pharmacy refills
Attend follow-ups and use messaging for questions. Your pharmacy handles dispensing; availability and prior authorization can affect when treatment starts.
How much does Form Health cost?
Brand-name Wegovy; medication extra. Self-pay care $299/mo; insured costs depend on benefits. Total ongoing price not published.
Brand-name Zepbound; medication extra. Self-pay care $299/mo; insured costs depend on benefits. Total ongoing price not published.
Not published — confirm location eligibility with Form before enrollment
The self-pay program is $299 per month for clinician and registered dietitian appointments, messaging and platform access. Lab work and medication are additional. Insured patients instead need a benefits check: appointment copays or deductibles and prescription costs depend on their plan. Neither route establishes a universal all-in monthly price for Wegovy or Zepbound.
- Ask for separate estimates for visits, any platform subscription, labs and the prescribed drug. The terms describe covered services that may be accessed without a subscription by contacting Form, with fewer platform benefits.
- Acceptance of your insurer does not establish coverage for your selected medication. Prior authorization support helps with the application but cannot guarantee the insurer's decision.
- Subscriptions renew automatically until canceled. The terms say cancellation closes the account at the end of the current billing period; already incurred charges remain payable.
- The terms separately request at least 15 days' notice to revoke recurring payment authorization. Confirm the cancellation deadline and any remaining invoices with billing rather than assuming the next charge will disappear.
- Manufacturer savings programs have their own eligibility rules. Form's savings guide does not guarantee a final pharmacy price.

What are Form Health's pros and cons?
Pros
- Registered dietitian visits give food choices and eating habits a defined place in treatment
- Monthly clinical follow-up provides a regular opportunity to discuss tolerability and changing needs
- The self-pay service fee clearly excludes medication and laboratory charges
- Patients can use their preferred pharmacy instead of relying on a single bundled dispensing arrangement
Cons
- The $299 self-pay fee can be substantial before paying for the medicine itself
- Benefits verification, records and pharmacy authorization add steps before a first fill
- Public pages do not establish a complete current state-availability list; confirm your location before enrolling
- Separate insurance claims, subscriptions and pharmacy bills require more budgeting than one bundled invoice
The pharmacy behind the medicine
It does not name its pharmacy. Form Health does not publicly disclose which compounding pharmacy fills its prescriptions. That is common and it is not an accusation — but it does mean the one thing you would most want to verify about a compounded medicine is the one thing you cannot.
A compounded drug is mixed to order by a pharmacy rather than manufactured and released as a finished product, so the FDA has not reviewed the specific preparation you are prescribed for safety, effectiveness or quality. That is true of every compounded program, including the highest-scoring ones.
What else do people ask about Form Health?
Does Form Health prescribe compounded GLP-1 medication?
No. Form says it prescribes only FDA-approved medications when appropriate. Its medication page lists Wegovy and Zepbound, alongside other prescription options. A clinician still determines whether a particular medicine is suitable.
Is medication included in the $299 monthly plan?
No. That self-pay fee covers program care, including clinician and registered dietitian visits, messaging and platform access. Medication and lab work are separate expenses.
Does insurance acceptance mean my GLP-1 will be covered?
No. Form can review benefits and submit prior authorization requests, but the insurer decides medication coverage. Check both your medical and prescription benefits before budgeting from a copay estimate.
Do I need a primary care doctor?
Yes. Form requires an ongoing primary care relationship and a visit within the previous 12 months. Enrollment includes records and a release so the care teams can coordinate.
Can I choose where my prescription is filled?
Yes. Form sends prescriptions to the patient's pharmacy of choice. Contact the pharmacy to confirm stock and your final price; receiving a prescription does not mean it is immediately ready to collect.
Can I cancel the program?
Form says membership can be canceled at any time. Its terms specify automatic renewal until cancellation and account closure at the end of the current period. Ask billing about the next processing date, payment authorization and any charges for care already received.
