Price transparency
What is disclosed before intake, not a clinical rating.
Shapely combines online weight-management visits, insurance navigation and ongoing support, with medication charged separately. Commercial-insurance members face a membership fee plus any visit cost-sharing; self-pay care has its own rate, and Medicare care is described differently. The program can make sense if you want medical follow-up and help using benefits, but the lowest drug headline is not the complete monthly bill.
Shapely does not pay us. We review it so you can compare it with the programs that do. Disclosure

The verdict
Shapely shows its price before the medical intake, but leaves gaps, such as what the advertised price includes or which pharmacy fills the prescription. It ranks 152 of 293 on price transparency.
The score beside Shapely's name rates how much it tells you before the medical intake — the price, the pharmacy and the terms — not how cheap it is.

How does Shapely work?
Check your state and benefit design
The homepage currently identifies California, Florida, New York and Texas for adults age 18 or older. Confirm the actual state and provider before selecting a plan. An announcement about eventual national expansion is not current coverage. Ask separately whether your health plan covers visits, dietitian services and the drug you are considering.
Choose insured or self-pay care
Commercial insurance, Medicare and self-pay are presented as different payment routes. A covered appointment may still have a copay, deductible or coinsurance. Ask for a written estimate for the membership, initial visit and follow-ups so the first bill does not depend on interpreting a general statement about accepting insurance.
Meet the provider over video
Submit medical history and discuss prior weight treatments, current medication and what support you need. A prescription remains the provider’s decision. Shapely recommends recent labs and accepts results completed within six months; if new testing is requested, ask whether your existing results are sufficient and what your lab or insurance will charge.
Set up pharmacy access and follow-up
Discuss branded coverage, manufacturer self-pay programs or a compounded option if clinically appropriate. The terms allow choosing your pharmacy. Ask who supplies medication and what happens if the prescription changes. Keep monthly follow-up and nutrition appointments visible in your calendar, and obtain the separate membership cancellation and pharmacy cancellation instructions before you need them.
How much does Shapely cost?
Compounded medication from $99/mo plus care plan/visit costs; ongoing month-to-month total not established.
Compounded medication from $166/mo plus care plan/visit costs; ongoing month-to-month total not established.
Available in California, Florida, New York and Texas; age 18+; confirm treatment-specific access before payment
Shapely’s commercial-insurance plan shows $49 per month plus copays, discounted to $9 for the first month. Self-pay care shows $99 per month, discounted to $49 for the first month. The Medicare option is presented as copays only. Medication is excluded from memberships. Compounded semaglutide is advertised from $99 per month and compounded tirzepatide from $166, but these separate starting drug figures do not establish a complete ongoing treatment total.
- The first-month membership discounts should not be carried forward into a recurring budget. Visit cost-sharing, the ongoing membership charge where applicable, and the actual pharmacy cost are separate amounts.
- Labs are excluded. The homepage lists $75 self-pay laboratory testing at Labcorp, while insured testing may have its own patient cost. Recent testing is strongly recommended rather than universally required before most medication starts.
- Medicare participation for care is distinct from coverage of a specific weight-management drug. Ask Shapely and your plan which service, indication and pharmacy benefit apply rather than assuming a Medicare care plan includes medication.
- Shapely’s public general terms allow additional program-specific agreements but do not establish a precise membership cancellation notice period or refund schedule. Obtain those terms in writing before authorizing recurring billing.
- The terms identify Zaghi Medical, Inc. doing business as Shapely, preserve pharmacy choice and state that prescriptions are discretionary. They also contain broad liability limitations and a contingent $100 cap if limitations are invalid or unenforceable, subject to applicable law.
- A compounded starting price does not specify your future supply, dose or dispensing arrangement. Ask whether higher doses, different pharmacies or the continuing care plan change the amount. Do not add two introductory headlines and label the result a standard monthly price.

Who is Shapely best for?
A good fit if
- Adults in the listed states who want video follow-up alongside help navigating benefits
- Insured patients comfortable checking separate visit and pharmacy cost-sharing
- People who want dietary support as part of an eligible insured care plan
Look elsewhere if
- Anyone outside the currently listed service states without explicit confirmation
- Shoppers expecting the membership fee to include medication and labs
- People choosing on the assumption that introductory care discounts recur indefinitely

What are Shapely's pros and cons?
Pros
- Commercial, Medicare and self-pay care routes are described separately
- Insured plans include recurring video and dietitian support
- Published lab and medication exclusions make the major budget components identifiable
Cons
- A complete total requires benefits verification and a specific pharmacy quote
- Public cancellation and refund details are incomplete for the actual membership
- Four-state availability limits access despite broader expansion language
The pharmacy behind the medicine
It does not name its pharmacy. Shapely does not publicly disclose which compounding pharmacy fills its prescriptions. That is common and it is not an accusation — but it does mean the one thing you would most want to verify about a compounded medicine is the one thing you cannot.
A compounded drug is mixed to order by a pharmacy rather than manufactured and released as a finished product, so the FDA has not reviewed the specific preparation you are prescribed for safety, effectiveness or quality. That is true of every compounded program, including the highest-scoring ones.
What else do people ask about Shapely?
What does the first-month $9 offer cover?
It is a discount on the commercial-insurance membership, normally shown at $49 per month, with applicable copays additional. It does not mean $9 buys a GLP-1 prescription supply. Medication, labs and other patient cost-sharing need their own estimate.
What does self-pay care cost?
The homepage shows a $99 monthly self-pay care rate, discounted to $49 for the first month. Medication and labs are excluded. Confirm the ongoing rate and plan-specific services before assuming the insured package and self-pay package include identical nutrition support.
Are the $99 and $166 drug prices total program prices?
No. They are starting cash-pay compounded medication figures for semaglutide and tirzepatide. The membership explicitly excludes medication. Your full bill depends on the care route, dose, pharmacy and any visits or testing you must pay for separately.
Where is Shapely available?
The homepage currently lists adults in California, Florida, New York and Texas. It describes expansion plans, but those plans should not substitute for actual location confirmation. Ask about your residence and the state where you will physically attend a telehealth visit.
Do I need new laboratory tests?
Shapely strongly recommends recent labs before most medication starts and says it can accept results from the prior six months. If testing is needed, it lists $75 for self-pay labs at Labcorp. Insurance coverage and any copay depend on the plan.
Can I cancel and get a refund?
The general public terms do not set out a clear membership notice deadline or complete refund policy. Ask for the actual enrollment agreement, renewal date and cancellation method before paying. Also ask when a pharmacy order becomes final, because membership cancellation may not stop a separate prescription purchase.
