Price transparency

What is disclosed before intake, not a clinical rating.

FormBlends facilitates online assessment and pharmacy delivery for compounded semaglutide and tirzepatide. Its pricing page separates introductory offers, one-time plans and an optional Subscribe & Save arrangement. That distinction matters because a long prepaid plan’s monthly equivalent is not a monthly payment, and the subscriber discount applies to the complete renewal period. A dedicated provider page names Receipt MD LLC as its independent clinical group. The platform also publishes specific remedies for nonprescribing and shipment problems. The pharmacy and selected plan still need confirmation, while a separate provider page identifies Receipt MD LLC as the medical group.

The verdict

Public prices describe specific visits, offers or commitments. They do not establish one unconditional ongoing monthly medication total.

The score beside FormBlends's name rates how much it tells you before the medical intake — the price, the pharmacy and the terms — not how cheap it is.

What do you get with FormBlends?

  • Independent clinical review of an online intake, followed by a prescription only if appropriate. Form Blends LLC describes itself as a platform rather than the treating practice, and the initial assessment is not a consultation until reviewed by a clinician.
  • Compounded semaglutide or tirzepatide with injection supplies and cold-chain shipping described as included. Laboratory work is excluded. Request the actual formulation and pharmacy label rather than assuming a product illustration establishes what will be dispensed.
  • One-time plans paid upfront, with optional Subscribe & Save. The pricing page says nothing renews unless the patient chooses that option; its renewal credit is a flat $25 against the plan-period price, not a monthly discount repeated within a longer term.
  • A published refund and replacement process covering clinical nonapproval, incorrect or compromised shipments and confirmed lost deliveries. These remedies have reporting conditions and should be kept separate from a general right to return dispensed medication.

How does FormBlends work?

  1. Choose a plan using total payments

The introductory month is $99 for semaglutide and $149 for tirzepatide. The ordinary one-month figures are $190 and $239, with optional subscriber renewals at $165 and $214. Longer plans are prepaid. Record the whole initial charge, whole renewal charge, supply duration and selected subscription setting so that a monthly equivalent does not obscure the actual commitment.

  2. Confirm clinical and state eligibility

The terms require adults aged 18 or older in a supported US state. Current coverage is restricted rather than universal. Ask whether the selected preparation and assigned clinician can serve you where you will be located. Provide your full history and prior treatment; a catalog entry or completed questionnaire does not guarantee a prescription.

  3. Ask who will prescribe and dispense

The current provider page identifies Receipt MD LLC as the independent medical group, while the actual dispensing pharmacy varies with preparation, state and capacity. Confirm your clinician and dispenser before fulfillment, together with added ingredients, concentration, supplied quantity and follow-up access. Separate the platform’s role from the professional making the medical decision and the pharmacy preparing the prescription.

  4. Record the next billing and reporting dates

If Subscribe & Save is selected, cancel before the next billing date shown in the confirmation. Obtain written acknowledgment. For a damaged, incorrect or questionable delivery, preserve photographs and report it promptly under the seven-day policy. Do not confuse a shipment remedy with permission to return an unwanted prescription after it has been dispensed.

Where it operates

Confirm clinical service and location eligibility. Selected US states and territories; current location and product eligibility must be confirmed

The current pricing page lists introductory months at $99 for semaglutide and $149 for tirzepatide, followed by one-month prices of $190 and $239 or optional subscriber renewals of $165 and $214. Longer prepaid plans have separate complete amounts and renewal credits. Laboratory work is excluded. The ongoing month-to-month total not established qualification reflects the need to select the actual plan, clinical requirements and full first and subsequent payments rather than rank the service by its introductory month alone.

  • Subscribe & Save is optional under the current pricing page. The flat $25 credit applies to the whole selected renewal term, so its monthly effect differs between short and long plans.
  • If the independent clinician does not prescribe, the policy provides a full refund. The published banking estimate is five to ten business days after issuance, rather than immediate availability of the funds.
  • Dispensed and shipped prescriptions are nonreturnable and nonrefundable under the policy. Ask about any unprocessed order promptly; stopping a future subscription is a separate action.
  • The policy offers no-charge replacement for reported damaged, spoiled or incorrect shipments and a tracing process for lost deliveries. Keep the actual order confirmation and evidence needed for the applicable remedy.

What are FormBlends's pros and cons?

Pros

  • The pricing page separates introductory and renewal amounts
  • Automatic renewal is optional rather than universal
  • The refund policy specifies shipment reporting conditions

Cons

  • Longer low equivalents require upfront payment
  • The actual clinician and dispensing pharmacy still need confirmation
  • Laboratory work adds a separate potential cost

The pharmacy behind the medicine

It does not name its pharmacy. FormBlends does not publicly disclose which compounding pharmacy fills its prescriptions. That is common and it is not an accusation — but it does mean the one thing you would most want to verify about a compounded medicine is the one thing you cannot.

A compounded drug is mixed to order by a pharmacy rather than manufactured and released as a finished product, so the FDA has not reviewed the specific preparation you are prescribed for safety, effectiveness or quality. That is true of every compounded program, including the highest-scoring ones.

What else do people ask about FormBlends?

Is every plan a subscription?

The pricing page says plans are one-time upfront purchases unless Subscribe & Save is chosen. Ask which setting is selected at checkout and save the confirmation. A one-time plan still commits its complete payment, while a subscription creates a future renewal on the selected plan’s schedule.

How does the $25 discount work?

Subscribe & Save deducts a flat $25 from the complete plan renewal price. It is not automatically $25 off each month inside a multi-month plan. Compare the whole renewed period and payment, then calculate an equivalent only after confirming the actual supplied duration.

Are laboratory tests included?

No. The pricing page excludes lab work while including provider review, prescribed medication, injection supplies and shipping. Ask whether your clinician expects testing and obtain its separate cost. Outside medical visits or tests are not covered simply because the medication package includes its own clinical assessment.

What if the clinician does not prescribe?

The refund policy says the complete charge is refunded to the original payment method if no prescription is issued. Keep the clinical and refund confirmation. The policy estimates that banks typically display an issued refund within five to ten business days, so the posting date can differ from the decision date.

Can I return a prescription I no longer want?

The policy excludes returns and refunds once the pharmacy has dispensed and shipped it. Contact the team before processing if plans change, and separately cancel any selected renewal. Do not assume an unopened package creates a return right; prescription orders follow a different process from ordinary merchandise.

What should I do if delivery is wrong or damaged?

Preserve the package, label and photographs and report the problem within seven days under the published remedy. The policy describes free replacement for qualifying damaged, spoiled or incorrect shipments, with a separate trace for lost deliveries. Ask the pharmacy before using medication you believe may be compromised.
